After discussion and consideration of the management, due to you are unable to fulfill obligations because of illness and according to your medical practitioner’s report it is unlikely that you will sufficiently recover to return to duty in the near future.
We regret to inform you that based on the ground of your medical report, the management has decided to discontinue your service due the concern of your health and safety. The aforesaid termination shall come into effect from (Date).
Under the Employment (Termination and Layoff Benefits) Regulations 1980, the company will compensate three (3) months salary to you which for an employee has been employed by the employer for more than 5 years service. We have been pleased with the qualities you have exhibited during your tenure of employment with us and will be sorry to lose you as an employee of the company.
