2nd Warning Letter

It has come to our notice that you have not seen performing your duties as you have been:

1) (Ex.: Absent without any notice given to the management on (Date). Staff shift 1pm but no call, try to contact but not pick-up phone.)

2) (Ex.: Next day (Date) inform at 11:22am by sms inform 2 days MC. After investigated and found out staff only entitled one day MC on (Date) (not Company panel clinic). Staff fraud MC on (Date).)

The Management is very disappointed with such uncalled as on several occasions. We take very seriously on this matter and issue with 2nd warning letter to you. Please be reminded that if you still do not show any improvement in your work, 3rd warning letter will be issued.
